INFORMATION FORM ON THE ACQUISITION AND PROCESSING OF PERSONAL DATA

Obtaining, Processing and Processing Purposes of Personal Data
Your personal data is collected and obtained verbally, in writing or visually or electronically, through call centers, websites, verbally or through other similar channels for the purpose of protecting public health, preventive medicine, medical diagnosis, treatment and care services provided by Health Agents Turkey.

Your health information: Your personal data relating to your health and sexual life obtained and collected during or as a result of the provision of medical diagnosis, treatment and care services, including but not limited to your laboratory test results, test results, medical examination data, appointment data and information, check-up data and prescription data and information; and

Compliance with legal and regulatory requirements and conditions; and
Sharing the requested information with private health insurance companies during the financing of health services; and
Conducting risk management and quality improvement activities through the Quality, Patient Experience and Information Systems departments; and
Invoicing by Patient Services, Financial Affairs and related departments for our services and confirming your relations with contracted institutions and companies; and
Participation in campaigns and sharing campaign information, designing and communicating special content, discrete and intangible benefits on web and mobile channels by the Media and Communications and Call Center departments.
Your personal data obtained and processed in accordance with the relevant laws and regulations may be transferred to Health Agents Turkey Clinic's physical archives and/or information systems, and may be kept and stored in both digital and physical environments.
- Transfer of Personal Data
Your personal data may be disclosed by Health Agents Turkey and its companies to third parties, including but not limited to private insurance companies, the Ministry of Health and its subdivisions, the Social Security Institution, the General Directorate of Security and other security forces, the General Directorate of Population and Census, the Turkish Pharmacists Association, courts and all other judicial authorities, the central etc. Third parties, including but not limited to third parties, your authorized representatives and agents and lawyers, tax and financial advisors and auditors, and our business partners and other third parties acting in cooperation with us for the further development or performance of health services in line with the aforementioned purposes, as well as regulatory and supervisory bodies and authorities and official authorities, will be processed in accordance with the Law and other applicable laws and regulations for the purposes of the aforementioned processing.
- Methods and Legal Grounds for Obtaining Personal Data
Your personal data is collected and processed in all kinds of verbal, written, visual or electronic media for the above-mentioned purposes and for the execution of all kinds of business within the field of activity of Health Agents Turkey within the legal framework and for Health Agents Turkey to fully and duly fulfill all kinds of contractual and legal duties and obligations in this context. The legal reasons for collecting and obtaining your personal data are the provisions of the relevant law:

In addition, as stated in paragraph 3 of Article 6 of the Law, personal data relating to health may also be processed by persons under the obligation of confidentiality or authorized official institutions and organizations, without obtaining prior explicit consent, only for the protection of public health, preventive medicine, medical diagnosis, treatment and care services, planning and management of health services and financing.
